ULTRAVIOLET RADIATION EXPOSURE
This lamp can cause serious skin burn and eye inflammation from shortwave ultraviolet radiation if outer jacket or envelope of the lamp is broken or punctured. Do not use where people will remain for more than a few minutes unless adequate shielding (tempered glass or other suitable enclosure materials described in paragraph 1 above) or other safety precautions are used.  Lamps that will automatically extinguish when the outer envelope is broken or punctured are commercially available.

ELECTRIC SHOCK HAZARD
Do not remove or insert lamp when power is on. If outer jacket is broken, shut off power immediately and remove lamp after it has cooled.

PROPER CARE AND MAINTENANCE

To minimize the hazards described above:

  1. Operate lamps only in compatible fixtures and on circuits wired with appropriate auxiliary equipment (See Operating Instructions below)
  2. Operate lamp only in position specified in Operating Instructions below
  3. Do not expose lamp to moisture
  4. Replace lamp if outer bulb is scratched
  5. Replace lamp at or before the end of rated life as specified in GTE Sylvania Product Bulletins for each lamp type
  6. Turn off for at least fifteen minutes at least once a week
  7. Electrically insulate any metal support in contact with outer jacket to avoid glass decomposition.
 
KEY TO DATE OF MANUFACTURE
The date of manufacture of this lamp is shown in coded information visible either on the metal tab inside the lamp near the base or embossed on the base.  The letters “SR” always precede the date code.  The numbers following “SR” represent a 3 digit, 10 year (120 month) date code starting with January 1981 as 001 and ending with December 1990 as 120.  For example, date code 007 designates a lamp manufactured in July 1981.

INSTALLATION AND OPERATING INSTRUCTIONS

Metalarc lamps will operate satisfactorily only if the auxiliary equipment (eg. Ballast, starter, capacitor, & socket) conforms to electrical specifications set forth in the Product Bulletins available from GTE Sylvania.  Metalarc lamps will not operate with auxiliary equipment designed for mercury lamps of the same wattage.

Lamps should be screwed into the socket firmly without using undue pressure.  Screw lamp into socket using dome end rather than the larger lamp diameter to avoid damaging the lamp.

This lamp is equipped with a date-recording base.  There is no guesswork about the burning life of the lamp. (Simply scratch through the appropriate code numbers, the month and year the lamp is installed).

The letters “BD”, “BU-HOR”, “BU”, “HOR”, or “U” appear on the outer jacket beneath the Sylvania logo, and specify the position in which the lamp must be operated.  Thee positions are as follows:

BD-Base Down, lamp must be operated only in a position which is within 15 degrees of Base Down Vertical.

BU-HOR Base Up to Horizontal, lamp must be operated in a position plus/minus 90 degrees of Base Up.

BU-Base Up Only, lamp must be operated in a position which is within 15 degrees Base Up.

HOR-Horizontal, lamp must be operated only in a position which is within 15 degrees of horizontal.

U-Universal, lamp must be operated in any burning position.
